Yoga is generally safe but can temporarily flare symptoms, so tailored, therapeutic programs and gradual progression are key.

A major Cochrane review of 21 randomized trials (2,223 adults) finds that yoga offers modest, but real, benefits for people living with chronic non-specific low back pain. Compared with no structured exercise (such as usual care, wait list, or back-care education only), yoga led to small improvements in back-specific function and pain after about three months, although average changes did not reach typical thresholds for “clinically important” relief. Many participants, however, were more likely to rate their back pain as “improved” or “resolved,” and physical and mental quality of life scores improved slightly. Most programs used modified Hatha, Iyengar, or Viniyoga, delivered in weekly group classes for 8–12 weeks, often combined with simple home practice. The review also shows that, when yoga is compared head-to-head with standard back-focused exercise (such as physical therapy or stretching/strengthening programs), differences in function and pain are small or uncertain suggesting yoga is a reasonable exercise option rather than a clearly superior one. Safety is an important nuance. Minor adverse events mainly temporary increases in back pain were more common with yoga than with doing nothing, but occurred at similar rates to other exercise programs, and serious events were rare. For clinicians and patients, this means that well-taught, back-specific yoga can be considered one potential component of long-term management, chosen based on preference, access, and cost, ideally starting with beginner or therapeutic-level classes and clear guidance to modify or stop poses that aggravate symptoms.
